Established as the State Hospital for Insane, Warren State Hospital admitted its first residents in 1880. By 1893, it was over capacity by at least 250 people. The hospital was established under the Kirkbride style of architecture and treatment, with an emphasis on on-site food cultivation. Its population peaked in the mid-twentieth century at about 2500 patients. Unlike many state run institutions, Warren State Hospital remains operational today, though at a much smaller size, with a capacity of around 200 patients.

The cemetery contains approximately 1,000 people, including veterans of various wars. Staff of Warren State Hospital undertook an extensive, two year cemetery restoration and memorialization project in 2006. This involved replacing numbered grave markers with new, concrete molded stones with engraved nameplates that listed names and birth and death dates (when possible to identify). Since the cemetery’s rededication in 2008, the staff hold an annual memorial and rededication service for the people buried there as well as former patients who died within the previous year.

The cemetery is open to visitors but located on hospital property. It is on the north side of Jackson Run Road, requiring a short walk from designated parking on the side of the road.
